Asked to bat at Warner Park, Trinidad & Tobago were reduced to 51-5 before getting bowled out for 137 by Jeremiah Louis (3-38) and Colin Archibald.
Leeward Islands captain Rahkeem Cornwall finished with 2-5 from three overs. His first wicket, of Tion Webster, was the 400th of his first-class career.
Kieran Powell (65) and Jewel Andrew (87) then took Leeward Islands to 318 as Anderson Phillip finished with 4-62 and Bryan Charles 3-81. Batting at eight, Cornwall made two runs.
Trailing by 191, Trinidad & Tobago began their innings on the second day and continued until the fourth morning. Ten of their batters reached double figures, but Jyd Goolie (64) was the only one to go past fifty, though Jason Mohammed made 49.
They were 68-1 when Cornwall came to bowl at opener Cephas Cooper. Cornwall tossed the ball up, and Cooper stepped out and drilled the ball down the ground, low and flat to Cornwall’s right.
Cornwall descended rapidly, and used only his right hand to come up with an incredible catch inches from the ground; he landed in front of Goolie. He subsequently trapped Mohammed leg-before and bowled Goolie to finish with 3-68, while Daniel Doram got 3-80.
Leeward Islands chased the target of 162 to win by four wickets. Cornwall remained unbeaten on two.
